Peer support through transition

Bridger Programs offer personalized peer support grounded in lived experience. Peer Bridgers walk alongside participants, build connections with community agencies and support goals defined by each person.

From discharge planning to community connection

Discharge planning is treated as a bridge to greater independence. The team supports transitions into the community and helps people reconnect with resources, opportunities and relationships that matter to them.

Practical support

Bridger staff can help people navigate applications and referrals involving SNAP, SSDI, reduced-fare MetroCards and other entitlements. The program also connects people with employment opportunities and legal services.
